Personal Data We Collect

Depending on the Service you engage us for, we may collect:

  • Identity data — full name, NRIC/FIN, date of birth, and Singpass-linked information necessary for LPA execution.

  • Contact data — home address, email, and phone number, including the address of any home or institution visit.

  • Medical and clinical data — information relevant to a mental capacity assessment, including relevant medical history, cognitive screening results, and our clinical findings and opinion.

  • LPA and donee data — details of donees, replacement donees, and the scope of the LPA, where relevant to certification or activation.

  • Payment data — processed via our third-party payment provider; we do not directly store full payment card details ourselves.

  • Technical data — such as IP address, browser type, and pages visited, collected automatically when you use our Website.

How We Collect Your Personal Data

We generally collect personal data:

  • directly from you, when you make an appointment enquiry, complete an LPA form, or communicate with us;

  • from your authorised representative — such as a donee, family member, or care institution arranging an appointment on your behalf — who we will treat as having your consent, or the appropriate legal authority, to share your personal data with us for that purpose; or

  • automatically, through your use of our Website.

If you provide us with personal data relating to a third party (for example, arranging a capacity assessment for a parent), you confirm that you are authorised to do so and have obtained any necessary consent on their behalf.

Purposes of Collection, Use & Disclosure

We collect, use, and disclose personal data for the following purposes:

  • performing our Services, including conducting assessments, completing certification, and submitting LPA documents to the Office of the Public Guardian (OPG);

  • verifying your identity, including via Singpass;

  • responding to enquiries, appointment requests, and feedback;

  • processing payment for our Services;

  • maintaining clinical documentation as required of a registered medical practitioner;

  • complying with applicable laws, regulations, or requests from regulatory or law enforcement authorities; and

  • operating, securing, and improving our Website.

We do not sell, rent, or trade your personal data to third parties, and we do not use your personal data for purposes incompatible with those stated above without your consent.

Disclosure of Personal Data

We may disclose personal data to:

  • the Office of the Public Guardian, where necessary to complete LPA certification, activation, or registration;

  • donee(s) named in your LPA, to the extent relevant to their role;

  • our third-party service providers, including payment processing, IT hosting, and encrypted document storage providers, who are contractually required to protect your data and use it only for the purposes we authorise;

  • our professional indemnity insurer, legal advisers, or the Singapore Medical Council, where necessary in connection with a claim, complaint, or regulatory matter; or

  • law enforcement or regulatory authorities, where required by law.

We will retain your personal data for the duration required by MOH or any related governing body (whichever is longer). Where personal data is no longer needed, it is securely deleted, anonymised, or otherwise disposed of.

Transfers of Personal Data

We generally store and process personal data within Singapore. However, some of our service providers (for example, our payment processor) may process data outside Singapore. Where this occurs, we take steps to ensure your personal data receives a standard of protection comparable to that required under the PDPA.
